I agree I am enrolling at Straight Blast Gym Naas (SBG Naas) and will be allowed to participate in the regular scheduled classes throughout the week as allocated to me and pledge to observe the rules and regulations of SBG and its instructors. I understand that the schedule is subject to change, that classes may be cancelled due to special circumstances, and that if I do not take a class within 30 days, I may have to re select my schedule which may be different depending on availability. I understand that I owe a month which will be paid with cash / card. If I am paying with credit card, I authorise SBG to debit my account, listed below,for the amount of the membership on the date listed above or thereafter. I understand that I am responsible for making sure that there are sufficient funds in my account or credit card on the day scheduled. In the case that a payment is not made, returned for insufficient funds , or is late I understand that a penalty fee of €20 may be billed to my account. The authorisation is extended by me to the SBG and /or its authorised agents or firms engaged in the business of processing and charging card debits. SBG has issued to me a membership number that must be used to log in prior to each class.
I understand that no payment or fees are refundable and that by signing this agreement i am waiving any free trials that I may be eligible for.
After the initial trial period, I understand the debit and membership will continue on a month to month basis at the agreed monthly rate using the billing information provided, until I give SBG two months paid notice of my intent to cancel. If you (the student) choose to end the month to month charging then decide to rejoin later, you will have to sign another contract. If you decide to cancel your account, it is required the you (the student) send your cancelation notice in writing, or hand deliver your notice and get a signed cancelation request from a SBG staff member, acknowledging the receipt of your notice. To cancel your account after the initial period, you MUST follow the above procedure.
- During the initial period of my contract, in the case that I am unable to attend classes at SBG, I understand that i will continue to be billed as agreed and that if I am missing a minimum of two months,I may receive credit for the time that I am gone by submitting a letter 7 days in advance , notifying a representative of SBG of the reason I will be gone and the dates of departure and return.
- You (the student) may cancel this agreement by midnight of the third business day after the date of this agreement (cooling off period). Excluding Sundays or Holidays, such cancelation must be in writing (email) or hand delivered to SBG. If you do hand deliver your cancelation notice, be sure to get a signed copy from a SBG staff member, acknowledging the receipt of your notice. If you miss the initial cancelation period, you may still cancel you contract afterwards ( see point 1). If your account goes more than 90 days delinquent you may be sent to collections for the full outstanding amount. You may cancel your membership if you develop a full disability, which prohibits you from utilising ALL portions of the gym, by having your physician fill out a disability form provided by SBG. A doctors note will also be accepted. For minor injuries, see paragraph above. You may terminate this agreement the event that you permanently move your residence more than 80kms away from our location. For termination due to a permanent move, a written notice of 30 days must be sent to SBG with legitimate verification of the move, any fees already collected are non-refundable, and a €50 termination fee will apply.
